Free Adult ADHD Self-Screen

This brief, research-based questionnaire (the ASRS v1.1) can help you decide whether a formal ADHD evaluation might be worth pursuing. It takes about 2 minutes. Your answers are scored privately in your own browser — nothing you enter is saved, sent, or seen by anyone, including this practice.

Please read first. This tool is for adults (18+) and is for educational and self-reflection purposes only. It is not a diagnosis and is not a substitute for a professional evaluation. Screening questionnaires are designed to be over-inclusive, so a “positive” result is common and does not mean you have ADHD — only that a fuller assessment may be worthwhile. A result below the threshold does not rule it out. If you are in crisis or thinking about harming yourself, call or text 988 (the Suicide & Crisis Lifeline) or go to your nearest emergency room.

Think about how you have felt and behaved over the past 6 months, and choose the answer that best describes you.

Please answer every question before viewing your results. The unanswered questions are highlighted above.
